Valvulotomy for Pulmonary
نویسندگان
چکیده
An earlier paper, reporting 3 successful cases (Brock, 1948), showed the feasibility of valvulotomy for the relief of pulmonary valvular stenosis. The present paper deals with our clinical and operative experience in 33 cases-a number large enough to allow certain conclusions to be drawn. Of the 33 patients submitted to operation, 18 had pure pulmonary stenosis, by which we mean pulmonary valvular stenosis with a closed interventricular septum, regardless of the patency or potential patency of the foramen ovale, and in 7 this was confirmed by necropsy (see Table I); all but 3 probably had a patent foramen ovale and in 6 this was proved by necropsy. In the other 15 (see Table II) valvular stenosis was thought to be part of a Fallot's tetralogy and in 3 this was confirmed by necropsy. Increasing experience in the diagnosis and surgical treatment of pulmonary stenosis has led us from a grave mortality of just over 50 per cent in the first 11 patients, most of whom were unlikely to live long as they had very large hearts and often congestive failure, to a mortality of 18 per cent in the remaining 22, these including many younger patients in whom thTe stenosis had not yet caused such serious changes;.

Late Hemodynamic Response to Correction of Isolated Pulmonary Stenosis by Open Operation during Pulmonary Bypass.
SURGICAL treatment of pulmonary stenosis has advanced to the point where a choice of several operative technics is available. This study constitutes a comparative review of the late recatheterization results in the open versus closed method of pulmonary valvuloplasty. Doyen reported an unsuccessful attempt to relieve pulmonary stenosis by transventricular valvulotomy in 1913.1 In 1948, Sellors ...

IN THE LAST 15 years surgical methods for the relief of the pulmonic obstruction in patients with pulmonary stenosis and intact ventricular septum have been successfully developed.1-6 Although the open-heart technic with the use of cardiopulmonary bypass is today the procedure of choice, the classic Brock's transventricular valvulotomy is still preferred for small infants.7-9 The final conclusi...
متن کاملHemodynamic Effects of Valvulotomy in Pulmonic Stenosis By PAUL
Seven patients with mild to moderate pulmonic stenosis with intact ventricular septum were subjected to cardiac catheterization before and from 4 to 18 months after pulmonary valvulotomy. Despite uniform clinical improvement, remarkably slight changes were observed, following surgery, in right ventricular pressure, pulmonary artery pressure, pulmonary artery flow, and pulmonary valve area, when...

متن کاملCritical pulmonary stenosis with a diminutive right ventricle in neonates.
Thirteen of 19 neonates seen in the first months of life with critical pulmonary stenosis (PS) and an intact ventricular septum had a diminutive right ventricle (RV) evidenced by electrocardiographic and angiocardiographic criteria. All were critically ill and at catheterization had hypoxemia secondary to right-to-left shunting at the atrial level, and RV hypertension. Right ventricular angiogr...
